This page contains a comprehensive collection of resources focusing on Advance Care Planning [ACP] and Health Care Consent [HCC] that are pertinent to practice of health care providers in Ontario.   These resources are generally informed by research studies, externally reviewed by experts and then adopted as a part of a framework to support health care providers in guiding patients through the Advance Care Planning process.

HCP-ACP-HCC Resource Navigator for Seniors, Families & Informal Caregivers

This paper focuses on common misconceptions or misunderstandings about the Substitute Decisions Act (SDA) and the Health Care Consent Act (HCCA).

This video reinforces the importance of Advance Care Planning.  Patients and health care professinals share their opinions and advice.

This one page handout describes what Advance Care Planning is in Ontario as well as the role of the Subsitute Decision Maker and Health Practitioners in regards to Health Care Consent.
