
Bridges to Care Long-Term Care Toolkits
PROJECT DESCRIPTION
The Bridges to Care Long-Term Care Project, funded by Health Force Ontario, was aimed at improving evidence-based knowledge-to-practice resources for clinicians within Long Term Care. Tools are grouped according to various practice settings including primary care, emergency care, acute care, community care, specialized care and long-term care. Tools are also organized alphabetically and by common geriatric issue.
After conducting an environmental scan and needs assessment for each pilot site affliated with the project, the resource development team developed tailored knowledge-to-practice resources for common care issues in long-term care. The resources were produced in a variety of print formats:
- Resource Manuals – Extensive collection of tools, assessments, care plans, algorithms and information sheets
- Resource Manual CD – Shipped with the Resource Manuals, the CD’s contain electronic copies all manual material
- Toolkits – Smaller, more concise collection of commonly used tools designed for quick reference
