Bone Fit™ is an evidence-informed exercise training workshop, designed for health care professionals (e.g. physiotherapists, rehab care staff) and exercise practitioners/trainers to provide training on the most appropriate, safe and effective methods to prescribe and progress exercise for people with osteoporosis. April 1, 2019 from 12-1PM EST join presenter Dr. Chris Frank, as he discusses communication strategies for having difficult conversations such as advance care planning, goals of care and end of life.  Learning objectives include understanding the difference between Advance Care Planning (ACP), goals of care and consent; incorporate ACP and goals of care discussions into regular outpatient care; using the principles to make CPR discussions easier for patients and you and incorporate Harvard Serious Illness Communication guide and other resources into your discussions.
