The electronic frailty interprofessional toolkit (eFit) includes one section for Health Care Professionals and one for Individuals & Caregivers. Each section begins with the topic of frailty and then expands to include other health conditions, issues related to aging, and healthy lifestyle topics. The toolkit provides evidence-based tools and resources. Please contact info@sagelink.ca with suggestions for additions or improvements.
The SE Regional Integrated Memory Program (RIMP) is an innovative outpatient program at Providence Care supporting individuals with cognitive disorders and their care partners with a specialized integrated approach. This program will provide diagnostic assessment, cross consultation, follow-up care and recommend evidence-based interprofessional care plans for individuals with dementia and their caregivers.
Each topic in the Health Care Professionals section contains a topic overview and assessment tools (e-form or printable PDF) with instructions on how to conduct the assessment and interpret results. There are also guidelines, additional resources and online learning to support health care professionals in the provision of clinical care.

The Southeast Regional Integrated Memory Clinic is a unique inter-professional service that operates out of Providence Care Hospital. It provides a comprehensive diagnostic assessment, cross consultation, follow-up care and access to supports and therapies for individuals with memory and related cognitive problems.
