Advance Care Planning

INTRODUCTION

After you have received a diagnosis of dementia it is recommended that you start thinking and talking with loved ones about your personal values and wishes, especially with respect to future healthcare, personal care, financial decisions and how you want to be involved in decision making. Talking about your wishes with trusted friends and family members when you are still able to do so can give you a sense of control over your future and put your family more at ease. Even though talking about these topics can be difficult, it is only by having open conversations with your designated Substitute Decision Maker that your wishes can be honored.
