Southeastern Ontario Portal for Advance Care Planning [ACP] & Health Care Consent [HCC]

PROJECT DESCRIPTION

Advance Care Planning (ACP) is a process which gives someone the opportunity to consider and discuss with family and friends their wishes and desires concerning future personal care options. This can be helpful in guiding their Substitute Decision Maker (SDM) if they ever become incapable of providing consent for health or personal care decisions themselves.  The Health Care Consent Act in Ontario requires Health Care Providers to obtain informed consent in order to give or withdraw treatment and ensures a process by which everyone in Ontario has access to a Substitute Decision Maker.

The Centre for Studies in Aging & Health developed a bilingual online portal to feature Advance Care Planning (ACP) and Health Care Consent (HCC) information that is important to Health Care Providers and people living in Ontario. The resources housed on that portal are now linked to on this project description page.

A number of Ontario specific educational resources were developed and disseminated as a lead member of the South East LHIN Advance Care Planning & Health Care Consent Working group. ACP & HCC RESOURCES FOR SENIORS, FAMILY & CAREGIVERS

This section contains a comprehensive collection of resources focusing on Advance Care Planning [ACP] and Health Care Consent [HCC] that are pertinent to seniors, families and caregivers in Ontario.   These resources are generally informed by research studies, externally reviewed by experts and then adopted as a part of a framework to support health care providers in guiding patients through the Advance Care Planning process.

This is a pdf copy of a presentation given by Mark Handelman for the Frontenac-Kingston Council on Aging about important considerations regarding capacity, consent and CCB hearings. Mark is one of Ontario’s first members of the official Guardian Child Representation Program. Mark was appointed to the Consent & Capacity Board in 1998 and became a vice chair and senior lawyer member in 2000. Research Paper for the Law Commission of Ontario: Health Care Consent and Advance Care Planning in Ontario
This is an executive summary that focuses on the interrelationship between health care consent and advance care planning under Ontario law, and on related misconceptions of health practitioners and health care organizations.  At common law and under Ontario legislation, informed consent is required before a health practitioner can provide treatment to a patient. Importantly, where a patient is found incapable, the requirement to obtain informed consent is not abrogated, but instead the informed consent is obtained from communications with substitute decision makers (SDMs).
