
Support for Ontario Health Teams in Caring for Older Adults
Co-developed with geriatric clinical experts from Ontario, on behalf of the Regional Geriatric Programs of Ontario, the sfCare Learning Series comprises introductory educational modules for clinicians, along with supporting posters and patient handouts on 7 key clinical topics: delirium, mobility, polypharmacy, pain, loneliness, nutrition, and urinary incontinence.
How can these modules and tools benefit clinicians, geriatric specialists, and healthcare organizations?
- Clinicians and geriatric specialists can use the posters and patient handouts as tools to support patient and caregiver engagement and patient self-management.
- Organizations can use the education modules to implement a new clinical training program or replace or refresh their current clinical training program.
- Clinicians in any sector who would like to learn more about providing excellent care for frail older adults can use the education modules for self-study.
A learning series is currently under development for caregivers as well as personal support workers. We will also be creating more advanced modules for clinicians.
